What Is a Compounding Pharmacy?

Compounding pharmacists provide custom medications when commercially available drugs do not meet patient needs. This may be necessary due to patient intolerance of inactive ingredients in the available drug; the patient needs a specific preparation that is unavailable; the patient needs a drug that is in shortage or discontinued.

Common reasons for compounding custom medication include:

  • To customize strength or dosage
  • To flavor a medication and make it more palatable
  • To eliminate unwanted, nonessential ingredients
  • To change the form of the medication (into a liquid, topical cream, suppository, or another form)

All pharmacists are trained in compounding, but most practitioners who specialize in this practice have undergone advanced training. About 7,500 pharmacies specialize in compounding services, of the approximately 56,000 community-based pharmacies in the United States. Compounding is regulated at the state level. Prior to the modern pharmaceutical industry, compounding was a routine activity for most pharmacists.
